By Mail - $15
Print out and complete the birth certificate request form.
Include these:
- Self-addressed, stamped envelope
- Copy of your driver's license or other ID
- $15 check or money order payable to "Vital Records"

Certificates available for:
- Deaths within the City of Kansas City, MO, starting in 1910
- Death records outside the city limits, but in the state of Missouri starting in 1980
In Person - $14 First Copy/$11 Each Additional Copy

*Who May Request and Receive Copies of a Birth or Death Certificate?
- The person named on the certificate
- A member of the immediate family
- Wife
- Husband
- Son
- Daughter
- Mother
- Father
- Brother
- Sister
- Grandmother and Grandfather
- Grandchild
- Aunt and Uncle
- Niece and Nephew (This list does not include cousins.)
- Others: guardian who shows guardian paperwork, foster parent with custody papers, step-parent and attorney acting on behalf of the registrant or family.
*Family members listed above who are qualified to receive a vital record may request a vital record when a qualified individual does not have identification. The family member must show their identification and must also be able to demonstrate a link to the person listed on the vital record.
